The information required in the BisonCare Health Portal will be: Student Health History, Physical Exam, Immunizations, a Tuberculosis Risk Questionnaire, and various consent forms. All Health Record Requirements are due by July 1st, 2024. Our community’s health and safety are top priorities and failure to meet the deadline with this requirement will result in a registration hold on the student’s account.

If participating in intercollegiate sports, NCAA regulations mandate that you have a physical within 6 months of the start of the sport you are involved in. In addition to the traditional health record requirements, athletes must submit sickle cell lab results and complete various consent forms on the BisonCare Health Portal.
